Conflict of Interest Filing Instructions

  1. Form 700 and Reference Pamphlet can be found at the California Fair Political Practices Commission Web site.
    1. 2011/2012 Form 700 (pdf)
    2. 2011/2012 Reference Pamphlet (pdf)
  2. Find your location and position title in the List of Designated Positions. See an explanation of the Disclosure Categories (pdf).
  3. Complete the cover page and all applicable schedules of the Form 700.
  4. Sign and date the cover page of the Form 700 in blue ink.
  5. Send the completed Form 700 with the original signature to:

    Annual forms are due April 2, 2012
