(a) Renewal of Passport

Requirements

(1) Passport original and photo copy
(2) Up to date receipt of income tax (For those holding 3 year validity passport. For 1 year passport its not applicable)
(3) 1 Biography Form                                                                                                                                 (4) Renewal fees $ 167 (Money order/Cashier check/cash only )                                                                        (5) Pre-paid self-addressed return envelope

Note : Myanmar Passport holders are required to renew their Passports before the date of expiry. A fine will be levied if the Passport is not renewed in time.

(b) Renewal of Myanmar Passports for Students

Requirements

(1) Application letter addressed to the Ambassador, Embassy of the Union of Myanmar, Washington, DC.
(2) Letter from the respective school/institution and I-20 Form                                                                        
(3) Photo copy of Passport ( every stamped pages )
(4) 3 Biography Form  ( 1 original and 2 extra copies )
(5) Pre-paid self-addressed return envelope

(c) Issuance of New Passport

Requirements

(1) Application letter
(2) Copy of all pages of the Passport
(3) Income Tax payment must be up to date when applying for a new Passport
(4) Two photographs
(5) 3 Biography Forms ( 1 original and 2 extra copies )
(6) Fees $ 300 (Money order/Cashier check/cash only )
(7) Pre-paid self-addressed return envelope

Note : Myanmar Passports holders and their dependents are eligible to apply for a new Passport when running out of blank pages.

(d) Country Endorsements

Requirements

(1) letter address to Ambassador, Myanmar Embassy, Washington, DC
(2) Original Passport
(3) Up to date receipt of income tax (if applicable)
(4) Fees $ 25 (Money order/Cashier check/cash only )

(e) Renewal of Certificate of Identity ( C of I )

Requirements

(1) Application letter by parents / guardian
(2) Up to date income tax receipt of parents/ guardian
(3) Photo copy of C of I
(4) Renewal fees $ 84 (Money order/Cashier check/cash only )
(5) 1 Biography Form ( If parents/guardian is tax payer)                                                                                  (6) 3 Biography Forms (For non-tax payers)                                                                                                   (7) Pre-paid self-addressed return envelope

(f) Issuance of New Certificate of Identity ( C of I )

(1) Application letter by parents / guardian
(2) Up to date income tax receipt of parents/ guardian ( if applicable )

(g) Attestation of Powers of Attorney

Requirements

(1) Revenue stamped document.
(2) Signature of 2 witnesses.
(3) Particulars of witnesses such as Passport No. NRC No, address must be stated.
(4) Person authorizing GP / SP must submit the photo-copy of his/her valid passport to the Embassy.
(5) Pre-paid self-addressed return envelope

( h ) Attestation of Certification

- Various documents such as Birth Certificate / Marriage Certificate / Degrees / Diploma / Driving license ..... etc           needed for attestation / certification is only done by submitting original document and those holding valid Myanmar passport.

- Processing fees $ 15 (Money order/Cashier check/cash only )

(i) Other Matters

For First Time Payment of Income Tax

Requirements

(1) Company letter stating employees' monthly basic salary.
(2) Application Letter addressed to the Ambassador, Embassy of the Union of Myanmar, Washington, DC
(3) Passport copy and visa pages (page showing entry date into the country)

(j) Renewal of 1 year validity Passport ( Social Visit )

The Embassy is not authorized to renew Passports with (1) year validity. The Embassy has to refer to the Myanmar passport Issuance Board to renew such passport.
 

Requirements

(k) Loss / Damage of passport

Requirements

(1) Application letter stating detail of loss / damage
(2) Police report ( for lost only )
(3) Copy of passport
(4) 3 Biography form 
(5) Two photographs
(6) Payment of new passport $ 300
(7) Pre-paid self-addressed return envelope

(l) Matters needed to be referred to the Passport Issuance Board, Ministry of Home Affairs, Yangon.

(1) Inclusion of new born child in mother's Passport
(2) Change of status (Social visit to job)
(3) Separation of child from mother's Passport and issuance of new Certificate of Identity ( C of I )
(4) Inclusion of a child who holds C of I to mother's Passport
(5) Change of status to dependent (foreigner husband or wife )

Note : The above mentioned cases need to be referred to the respective Ministry in Myanmar. Hence, it is advisable to submit in advance.
