UCOP IT Services - Employee Separation/Account Cancellation

Use this form for Employees or Contractors separating from UCOP. It is used for cancelling network access, E-mail/Calendar, TSM, VPN, BlackBerry server access, and Corporate Systems Gateway accounts. For assistance, contact Technology Service Desk. Updates to services normally take affect within two working days after the form is submitted, unless otherwise specified.

Employee access is terminated on effective date. Any out-of-office message in place prior to the effective date will remain active for 45 days. Also, the employee's entry in the Outlook address book will appear during this period. See Key Points on Use of UCOP E-mail Accounts for policy information.

Managers should work with employees to properly relocate or dispose of all computer files and e-mail records prior to employee separation. See the UCOP guidelines on accessing employee e-mail and files for more information.
