As departments and functional areas reorganize, it’s important to update contact information, make sure employees have access to the IT services they need to perform their jobs, and cancel unnecessary services. Please use the online forms listed below to submit requests.
UCOP Directory: Everyone can check to make sure their title, phone/fax numbers, and location are correct. If not, use the form to submit changes.
Add or Change IT Services: Managers should submit requests for e-mail/calendar, network access, and backup services for new employees, or when current employees change departments.
Cancel IT Services: Managers must cancel IT accounts when employees separate. Remember to copy electronic files as necessary before the individual’s last day at work. An e-mail out-of-office message may be set up before the services are cancelled and will remain in effect for 45 days.
Add or Cancel Phone Services: For office and cell phone services, which require departmental recharge, the iRequest system must be used.
